I'ELOISE M. DUNN, as Executrix of the Will of SYLVANA PIEDAD JIMENEZ,
deceased, pursuant to the Order of the Superior Court of the State of
California, for the County of Orange, made in the matter of the Estate
of SYLVANA PIEDAD JIMENEZ on March 18, 1983, confirming sale of real
property and directing the execution of a conveyance, hereby convey to
the CITY OF SAN JUAN CAPISTRANO, a Municipal Corporation, without any
representation, warranty, or covenant of any kind, express or implied,
all right, title and interest of decedent at the time of her death and
all right, title, and interest that the estate may have subsequently
acquired in the real property situated in the City of San Juan Capistrano,
County of Orange, California, described as follows:

dollars ($100), or (2) 10 percent of the taxes applicable to the new base year value reflecting the change in ownership of the real property or
mobilehome, whichever is greater, but not to exceed two thousand five hundred dollars ($2,500) if such failure to file was not willful. This
penalty will be added to the assessment roll and shall be collected like any other delinquent property taxes, and be subject to the same penalties
for nonpayment.

Revenue and Taxation Code (Rev. & Tax.) Section 480(a) requires the Assessor to obtain certain information
from property owners whenever there is an ownership change or any other change affecting title. This form is
a written request from the Assessor for a Change in Ownership Statement. When completed, signed, and return-
ed, it will constitute your compliance with the law. Failure to file the statement will result in the levy of a
penalty as required by law.
