Proposed Increase in Assessment Per Parcel:
Scaled increase as follows: FY 2008-2009 by 15% and FY 2009-2010 by 10%, with future annual
assessments being increased per the cost of living each year, on an as needed basis to be
determined by the City, not to exceed 50% of operating reserves, according to the change in
Consumer Price Index (CPI), All Urban Consumers, for the Los Angeles, Riverside, and Orange
County areas.
The person completing and submitting this assessment ballot must be the record owner of the property identified
above or the representative of the record owner of such property who is legally authorized to complete and submit this
ballot for and on behalf of the record owner. Please see “Summary of Assessment Balloting Procedures” on the back
of this ballot to assist in filling out the ballot. If there are two or more property owners, only one needs to sign and
return the ballot.
Please mark the box and fill out your ballot in ink. Do not use pencil.
Upon completion, fold the ballot, place it in the return envelope and seal the envelope. Mail or deliver the assessment
ballot to the address shown on the return envelope pursuant to the instructions on the back of this ballot sheet.

SUMMARY OF ASSESSMENT BALLOT PROCEDURES
If you are the owner of the property described on the enclosed ballot, or the authorized representative
of the record owner, you must submit the enclosed ballot to the City to support or oppose the
proposed assessment increase. Please follow the instructions below to complete and return your
ballot.
1. Register your vote on the enclosed ballot in favor or against the proposed assessment by
placing an “X” in the corresponding box.
2. Mark, sign, and date your ballot in pen. (Assessment Ballots received without a signature will
not be counted.) Do not use pencil.
3. Please use the enclosed return envelope to mail your ballot. Postage has been provided.
4. Seal and mail or personally deliver your ballot to the City Clerk of the City of San Juan
Capistrano, 32400 Paseo Adelanto, San Juan Capistrano, CA 92675. The City must receive
mailed assessment ballots by July 14, 2008. Postmarks will not be accepted.
5. Assessment Ballots must be received by the City Clerk prior to the close of the Public Hearing
concerning the proposed assessment on July 15, 2008, at the San Juan Capistrano City Hall
Council Chambers. Any ballots received after the close of the Public Hearing cannot legally be
counted.
6. Following the close of the Public Hearing, the City Clerk, or designee, will tabulate the
assessment ballots received.
7. The assessment may be confirmed unless a majority protest exists. A majority protest exists if,
upon the close of the July 15, 2008, Public Hearing, the number of ballots submitted in
opposition to the Assessment exceeds the number of ballots submitted in favor of the
Assessment.
8. If a majority protest exists based upon the ballots received by the City Clerk prior to the close
of the July 15, 2008, Public Hearing, the assessment increase will not be imposed.
Additionally, the service level of work proposed will be reduced to a level that can be
maintained by the current assessment.
9. Your assessment ballot is not confidential and may be subject to public disclosure.
10. The Engineer’s Report prepared in connection with the levy of the assessments for FY 2008-
2009 is on file in the office of the City Clerk and open to public inspection. The office of the
City Clerk is located at 32400 Paseo Adelanto, San Juan Capistrano, California.
