Located in the Very High Fire Hazard Severity Zone (VHFHSZ), SRA Moderate-High, Ember Zone 2, Special Fire
Protection Area (Maps at Building Department, OCFA, and City and OCFA Websites)or within *Fuel Modification Zones
Site Plan required:
Submit (2) site plan exhibits, hand drawn or professionally drawn.
Show proposed structures labeled with a number, when more than one proposed structure.
Show the proximity in feet from the proposed location(s) to the; back and the side property lines,
distance from the home, and distance from the ridge or base of slopes in or adjoining your yard.
The plan submitted to OCFA shall match the plan provided to the Building Department.

Revised Architectural Request ~ Landscape & Pergola
Dear Homeowner:
The Architectural Review Committee reviewed your architectural application, received in our office for:
Revised arch application. I have attached the additional items for the pergola: Images, specifications,
and color samples (bronze/tan) for 10’ x 16’3” louvered pergola by Outdoor Elements. All
other items remain the same - Renderings and specifications, color (pictures - honed nickle) for the
Fire Pit. Will be built to plan already submitted. Turf samples for putting green and surrounding turf.

mixture of Agave, Italian Cypress, Iceberg Roses and Olive Trees.) Hardscape will be concrete and
porcelain tile as pictured in renderings. Designer - Yardzen. Customer Rep - Emma 817.697.0405
Contractor - Salina’s and Paribas Construction 949.212.4471. Please let me know if there is any
additional information you need other than the two requested checks. Your application has been approved
as submitted.
This modification must be completed within 90 days; failure to comply may result in you being required to
resubmit your application. All work and future upkeep is to be done at your expense and shall be performed
at a time and in a manner to minimize interference and inconvenience to other homeowners. You shall
assume all liability and will be responsible for all damage and/or injury that may result from performance
of this work. If applicable, the required architectural deposit will be held pending completion and a final
inspection by the architectural committee to ensure all requirements were met and common area(s) of the
association was not damaged.
When the work is completed, please complete the attached “Notice of Completion” form and return to
Optimum Professional Property Management, Inc. Upon Receipt of the Notice of Completion, the
Architectural Committee will be notified that the above referenced modification has been completed and is
ready for inspection.
Furthermore, you are responsible for complying with all applicable federal, state and local laws, codes and
regulations and requirements in connection with this work, and shall obtain any necessary governmental
permits and approval for the work. The Board of Directors, its agent and the Committee have no
responsibility with respect to such compliance and the Board of Directors or its designated Committee's
approval of this request shall not be understood as making of any representation or warranty that the plans,
specifications or work comply with any law, code, regulation or governmental requirement.
