1.0 INTRODUCTION
Pursuant to your request, Inland Engineering Technologies, Inc. (IET) is providing this report presenting the
infiltrometer testing for the San Juan Rios District located at Paseo Adelanto & River Street, San Juan
Capistrano, California. The purpose of our infiltrometer testing was to determine the infiltration rates and
physical characteristics of the subsurface soils existing within the subject site. It is our understanding that a
storm water infiltration system is preliminarily proposed at the site.
2.0 INFILTRATION TESTING
On June 30, 2016 infiltrometer testing was initiated at the site to evaluate the infiltration rate in general
accordance with ASTM D 3385-03 -Standard Test Method for infiltration rate of soils in field using Double-
Ring Infiltrometer. Infiltration testing elevation was approximately 5 feet below ground surface. The infiltration
testing was initiated by filling both inner and outer rings with water to approximately 6 inches and allowed to
drain. Before recording each set of reading, both outer and inner rings were filled again with water to
approximately 6 inches. Initial and final water levels were recorded every 15 minutes for the first hour and
every 30 minutes thereafter. The infiltration test data recorded in the field are presented in Table 1 below.

Purpose
This procedure establishes the minimum requirements for the lockout of Siemens Main Switch Board (MSB-GH),
20004 Pull Section and corresponding Main Circuit Breaker to include sub panel HPGH to ensure equipment is isolated
from all potentially hazardous energy sources and locked out from unexpected energization.
Responsible Party
Millcreek Electrical, Inc. assumes full responsibility for the process of lockouts, tag outs and safe off of all equipment
denoted above.
Accessibility to Electrical Room/Equipment
Access to the electrical room, Siemens Main Switch Board (MSB-GH), 2000A Pull Section, Main Circuit Breaker and sub
panel HPGH will be limited to Millcreek Electrical, Inc. employees only. Millcreek Electrical, Inc. will possess the lock
out keys at all times and furthermore, will ensure that locks are not be removed from any equipment/breakers until
approved by the City of San Juan Capistrano Electrical Inspector.
Preparing Equipment for Energization
When the servicing is completed and the machine or equipment is ready for normal operating condition, the following
steps shall be taken.
( 1 ) Secure approval from City of San Juan Capistrano Electrical Inspector for removal of lock(s) from energy
isolating device(s) to include but not limited to the Siemens Main Switch Board (MSB-GH), 20004 Pull Section,
Main Circuit Breaker and sub panel HPGH.
(2) Check the machine or equipment and the immediate area around the machine to ensure that nonessential items
have been removed and that the machine or equipment components are operationallyintact.
(3) Check the work area to ensure that all employees have been safely positioned or removed from thearea.
(4) Verifu that the controls are in neutral.
(5) Remove the lockout devices and energize the machine orequipment.
