5. The City Engineer may, either at the time of permit issuance or at any time thereafter until completion of work, set additional conditions necessary
for the protection of the street or for the prevention of undue interference with traffic or to assure the safety of persons using the street.
6. This permit may be revoked at any time for any violation of this permit and all related conditions or when the public health, safety, or welfare
requires such revocation.

8. All trenching, excavation and backfill in the right-of-way shall comply with accepted City Standards and as accepted by the City Engineer.
9. No trenches or openings in street shall be left open during non -working hours. All trenches (or openings) shall be filled and covered with temporary
AC or steel plates secured with spikes and temporary AC feathered edges to the satisfaction of the City Engineer.
10. Permittee is required to place a Type II Slurry Seal the full lane width of all lanes in which excavation is made. In cases where the street had been
sealed during the previous 12 months, sealing is required only on the patch area. In cased where the City has programmed the streets to be
sealed during the next 12 months, permittee shall pay the City for the cost of the slurry in lieu of doing the work.
11. Permanent pavement repairs shall be made within 30 days after excavation work is completed. Permittee is responsible for replacing all traffic
striping and pavement markings.
12. After completion of work, all excess materials, shall be removed from the right-of-way and work area returned to original or better condition.
13. Damage caused by contractor to any infrastructure, utility, public or private improvements, etc. shall be replaced/restored as directed by the City
Engineer, or his designee at the sole expense of the contractor.

14. Traffic Control shall be provided and maintained per an approved traffic control plan. All traffic plans are subject to modification by the City Traffic
Engineer at any time during work under this permit. The current edition of WATCH or Caltrans Manual of Traffic Control for Construction and
Maintenance Work Zones is the standard accepted by the City.
15. All lane closures shall be by City approved traffic control plans with two-way traffic maintained at all times. Permitted hours of lane closures are
from 8:30 am to 3:30 pm and shall be clear of all obstructions and completely open to traffic at the end of each working day.
16. Contractor shall maintain a clearly marked, unobstructed access passed the work area. At no time shall pedestrians or bicyclists be diverted into
a portion of the road used for vehicle traffic. Access to commercial areas shall be maintained at all times.
17. Closure of On -street Parking - Contractor shall place temporary "No Parking" signs 72 hours prior to work and shall remove said signs upon
completion of closure(s). Signs shall clearly state date(s) and hours of closures.
18. Contractor shall insure the maintenance of signing/striping and traffic control devices at all times. Signing/striping/traffic signal work only be
performed by a contractor deemed mutually acceptable to the City of San Juan Capistrano.
19. Any damage to traffic control equipment shall be the responsibility of the permittee and shall be brought to the immediate attention of the
Engineering Inspector.
ADDITIONAL CONDITIONS AND INFORMATION - - -
A. Inspection Fee: At the time of permit issuance, inspection fees shall be required (along with an issuance fee and deposit). If additional inspection
time is necessary, those additional fees shall be deducted from the deposit.
B. Schedule of Work: Contractor shall provide the City with an estimated schedule of work for all work of three (3) or more days duration or as
requested by the Engineering Division.
C. Insurance: A current, valid certificate of insurance showing proof of workers compensation and general liability in the amount of One Million
dollars, with the City of San Juan Capistrano named as additional insured, must be on file with the City.
D. Arohaeological Monitongg: A City -approved, certified archaeologist shall be required on-site during excavation activities if project area is
determined to be in an archaeological sensitive area.
E. Haul Routes: It shall be the responsibility of the contractor to keep the City streets clean of any dirt or debris being imported to, or exported from„
the job site. Any materials spilled on the street shall be cleaned immediately upon occurrence.
F. Curb Cores: Max. diameter of pipes shall be 4" o.d. for 8" curb face and 3" o.d. for 6" curb face. Clearance between pipes shall be 12" min. and
12" off any construction joint. All cores to be in full curb height. SJC Standard Plan 355.
G. privewav Approach Removal- Saw cut all concrete and asphalt in dean straight lines at the nearest joint".
H. Drivewav Approach Construction: For curb and gutter removal, saw cut join of asphalt and gutter. If edge of asphalt is damaged during
oonstruction, contractor shall remove and replace 3'-0" wide AC/AB section, SJC Standard Plan 400-403.
I. Sidewalks: Residential sidewalks adjacent to curb shall be constructed per SJC Standard Plan 710. WPJ shall be placed at each side of
driveways, at all curb returns, and at 10'-0 intervals. Thickness of slab = 4" PCC standard. All sidewalks dr shall be joint to joint.
J. Monumentation: A licensed Land/Civil Surveyor is required to set new Monumentation/re-establish damaged or destroyed monumentation during
construction/excavation in the City right-of-way. Monuments and Comer Records shall be submitted to the City and filed with the County Surveyor
in compliance with AB 1414 and Section 8771 of the Business and Professional Code.
K. Regional Water Quality Control Board - In accordance with Sec. 14 of the SJC Municipal Code, Contractor shall not cause construction
materials/waste from road paving/ surfacing/ashpalt and concrete removal to discharge to storm drain system and/or watercourses. Saw -cuts -
Contraclor shall contain slurry residue before runoff drains to gutters, storm drains, catch basins and/or watercourses and shall remove all waste
residue from the site each day. Prohibited discharges shall terminate this permit.
BEST MANAGEMENT PRACTICES/NPDES RESIREMENTS
Attachment to Encroachment Permits for Work in the Public Right -of -Way
1. Identify (show) nearest downstream catch basin and/or storm drain inlets and provide protection around
it, gavel bags or other method approved by City, during the entire construction phase. Protection must
be in place and maintained during the entire construction process.
2. Provide at catch basin and/or storm drain inlets a way to catch debris from entering inlets and provide
protection around it, gravel bags or other method approved by City, during entire construction phase.

3. Provide protection during entire construction phase around all inlets, gravel bags or other method
approved by City, where soil is not stabilized yet with vegetation.
4. Provide sediment trap at outlet of pipe, prior to entering v -gutter during entire construction phase. Rip
rap may be used if appropriate - check State manual, ESC 32 pages 5-47 to 5-52. Add detail to plan.
5. Provide additional gravel bags or straw bales on-site, especially in areas with steep slopes, to slow
water and provide additional sediment control during entire construction phase.
6. Provide two row high gravel bags or straw bales on the slope downside next to the proposed silt fences.
7. All slopes should be stabilized immediately after construction (such as hydro seeding, or other approved
measures) and inlets should be protected until vegetation is at least 80% established.

9. A Stabilized Construction Entrance must be set up prior to any grading or earthwork operation. Add
detail to plan (Best Management Practices Handbook, ESC24, Page 5-39).
10. When saw cutting, all water shall be vacuumed.
11. During concrete work, provide concrete wash area on-site, out of right-of-way.
12. During construction, the Permittee must provide daily street sweeping.
13. City Engineer or his designee (the Engineering Inspector and/or NPDES Coordinator) may require
additional erosion control as he may deem reasonably necessary to meet NPDES requirements.
14. Permittee shall provide best practices operations to prevent construction debris and pollutants from
discharging to gutters, storm drains, and watercourses. In the event of rain, immediate erosion control
measures to prevent silt and debris from construction materials/spoils from flowing onto the public way
or entering in to the public storm drain system. These measures shall include, bOt shall not be limited
to, placing plastic covering over stockpile, creating a silt detention system prior to any storm drain inlet,
gravel bag placement, etc.
15. In the event of rain, the Permittee shall provide immediate erosion control measures to prevent silt and
debris from the stockpile site from flowirtq onto the public way or impeding the public storm drain
system. These measures shall include, but shall not be limited to, placing plastic covering over
stockpile, creating a silt detention system prior to any storm drain inlet, sandbag placement, etc.
